Submit completed application forms with the application fee by mail (preferably priority mail or express) to:

If you specify a valid e-mail address on your application, you will receive automated notifications when the required Transcripts and Letters of Recommendation are received at our office.

There is a $200 dollar processing fee, which must accompany your signed application. There are no registration fees.
